If you need forms notarized, we can do so in the office! Dr. Patel and Mrs. Patel are both Florida Notaries. If your child is doing after school sports, some of those consent forms may need to be notarized. We can do this in the office to make life a little easier for parents.
Notaries can be done walk-in or at your office visit! Please bring your ID!
